Thank for your interest in applying for the JAINA Academic Scholarship Program.

Please fill out the application below, complete the written essay, and return all documentation to the address provided on the application form. Applications must be postmarked by May 31, 2015.

Applicants must submit the application along with a high school transcript, letters of recommendation, and an acceptance letter from an accredited institution of higher learning which must be a non-profit educational institution. Submission of applications online with the necessary attachments is highly encouraged.
